Short version: the only thing this website collects is what you type into the appointment form, and we use it only to book your visit.

What this notice covers
This notice describes how the Davey Family Dentistry website (davey.ibadesign.co and any other domain that serves it) handles information you share while using it. It is separate from the Notice of Privacy Practices you receive as a patient, which governs your dental records under HIPAA.
Information you give us
The only place this site asks for information is the appointment request form. It collects your name, phone number, email address, the service you are interested in, a preferred date, and anything you choose to write in the message field. We use it for one purpose: to contact you and schedule your visit. Please do not include detailed medical history in the form — bring it to your appointment or call us instead.
How the form is delivered
Requests are sent to the practice's email inbox. Depending on how the site is configured, that happens either through an email delivery service acting on our behalf, or through your own email application opening a pre-filled message. Either way, the request is read by our front-desk team and kept only as long as needed to schedule and follow up on your visit.
Information collected automatically
Our hosting provider records standard server logs — such as your IP address, browser type, and the pages requested — to keep the site running securely. The map on the contact page loads tiles from a mapping provider, which receives your IP address and the map area requested in order to draw it. We do not run advertising trackers, and we do not sell or rent any information about visitors.
Cookies
This site does not set cookies for tracking or advertising. Any cookies present are strictly technical ones set by the hosting platform to serve the site.
Links to other sites
Links to Google Maps, Google reviews, and similar services take you to sites we do not control. Their own privacy policies apply once you leave this one.
Children
The appointment form is intended for adults booking on their own or a family member's behalf. We do not knowingly collect information from children through this website.
Your choices and questions
